Dates: June 10,17, 24, July 1,8,15,22, August 5,12
All physicals are by appointment ONLY! All students entering Pre-K, K,6,9 grades MUST have IL school physical exam & immunizations up to date. Charges will be billed to private insurance or Medicaid when available.
Immunizations available a basic fee will be charged at the time of service to those without insurance.
