General Athletic Information

All athletes must have the following on file to try-out for any athletic activity:
   Current Physical


Once the team is determined, the following paperwork must be on file prior to the first game of the season:
   Medical Release Form
   Code of Conduct
   Insurance Waiver
   $75.00 Activity Fee
